NEW PATIENTS

Thank you for choosing Dolan Family Dentistry! In order to make your check-in smoother and see you in a timely manner, please complete the six forms in our welcome packet. Once complete, you can use the “contact us today” on the right side of this webpage to request an appointment. If you need us to request your records from a previous dental office, please also complete the records release at the bottom of this page.

A few tips:

• Don’t forget to request an appointment with us if you have specific days or times that work best! Use the green box on the right & we will call you with availability.
• If you are completing the packet for more than one person, you may need to clear your browser before starting the forms for the next person. It’s best to do all the forms for one person, then clear the browser and complete them for the next person.
• If you have filled out your forms and haven’t heard from our scheduling staff after a few business days, please reach out to us!
