NEW PATIENTS

Thank you for choosing Dolan Family Dentistry! In order to make your check-in smoother, please complete the six forms in our welcome packet. Once we are able to review your forms and set up a chart, we will call you to schedule. If you need us to request your records from a previous dental office, please also complete the records release at the bottom of this page.

A few tips:

• If you are completing the packet for more than one person, you may need to clear your browser before starting the forms for the next person. It’s best to do all the forms for one person, then clear the browser and complete them for the next person.
• If you have filled out your forms and haven’t heard from our scheduling staff after a few business days, please reach out to us!
• Our office utilizes text, email, and phone calls to confirm appointments. Please make sure we receive confirmation by 2:00pm the business day prior to your appointment. If we do not receive confirmation by that time, the appointment will be removed from the schedule and the next person on our waitlist will be offered the appointment time.
